THE MINISTER OF HEALTH AND FAMILY WELFARE (DR. ANBUMANI RAMADOSS): Sir, under the Pradhan Mantri Swasthya Suraksha Yojana the Government of India intends to start six new AIIMS like institutions in States like Bihar, Madhya Pradesh, Orissa, Rajasthan, Uttarakhand and Chhattisgarh. The CCA had given approval only in 2006. Since then we have been going through a lot of process. One tender was un-responsive. Another tender was a single bidder. We had to go through the project management consultants. In all we have to go through a lot of process and there was a delay. I accept the responsibility for delay on part of my Ministry. Nevertheless, I had again asked them to quicken the process. Finally, I could say that the construction work has started in five out of six AIIMS like institutions, including Bihar, Orissa and Rajasthan. Except Bhopal, work at other five sites has already begun and at Bhopal the work will start in the beginning of November.
I categorically assure that the work has started and the entire construction work will be over in another two-and-a-half to three years and it will be fully functional. Not only that, we also want to upgrade some more institutions in Northern part of the country. We have added some more institutions, which again after getting the approval we will inform the hon. House.

Each of the institution would train 100 undergraduates and similar number of PG/PhD students every year. The medical Education consultant will help the Ministry in the curriculum design and training that is appropriate in a state –of-the-art Medical Education, using latest tools of technology. The consultant will indicate best practice to ensure adequate resource pooling and networking between the six AIIMS-like institutions and other existing GOI institutions. Prime Minister, Manmohan Singh is expected to announce the "much-awaited" scheme to upgrade Amritsar Medical College under the Pradhan Mantri Swastha Suraksha Yojana (PMSSY), Phase II, during his visit to Amritsar starting on September 15.
It is learnt that Punjab Chief Minister Parkash Singh Badal, who has long been pressing this demand, recently met the PM for this. Amritsar is one of the five medical colleges that the Union Health Ministry has listed for upgradation under Phase II, the others being Aligarh, Nagpur, Himachal Pradesh, Tamil Nadu, besides two AIIMS-like institutes in Uttar Pradesh and Bengal. "The proposal has gone to the Expenditure Finance Committee (EFC) and will be soon put before the Cabinet," a senior official told The Indian Express.
… While the cost of upgrading each existing site is Rs 150 crore, the AIIMS-like institutes will require an investment of Rs 750 crore each. For Phase I, meanwhile, one institute each will be set up in the states of Bihar (Patna), Madhya Pradesh (Bhopal), Orissa (Bhubaneswar), Rajasthan (Jodhpur),Chhattisgarh (Raipur) and Uttarakhand (Rishikesh).

Even though the necessary funds were approved for the Phase I sites in 2004-2005, they are yet to be completed despite the 2008 deadline. Although work on each centre has started, the Government has extended the deadline by two years.
The delay has come at a heavy price. Initially, the approximate expenditure was Rs 280 crore for each institute, but "in March 2006, financial projections were made and it was set at Rs 330 crore each. This is expected to go up to at least Rs 450 crore, as the prices of construction material have gone up," said a senior official in the Health Ministry. Officials blame the poor response of developers to their tenders as a major cause of the setback.

ANBUMANI RAMADOSS: … Coming to the AIIMS-like institutions, my predecessor, Shrimati Sushma Swaraj, when she was the Minister, she had launched a scheme called Pradhan Mantri Swasth Suraksha Yojana, under which they wanted to build six new AIIMS-like institutions and upgrade seven existing institutions to a certain level. Then, after that, our Government took over. At that time, since elections were there, they could not proceed further and the UPA Government took over. It is not like that with the change in Government, the policy would change. Shri Tripathy was mentioning about the political interference and all that, which I would definitely counter that. It is a continuity. Government comes and Government goes but it is a continuity. Definitely, we are following the programme. I would like to commend my predecessor, Shrimati Sushma Swaraj for bringing out that very innovative concept. I have already thanked and commended her for this. Definitely, we are going through this. Again, I would like to say that there has been a delay. I acknowledge the delay; I regret for the same. I could explain the delay, etc. In fact, the Standing Committee on Health has gone in depth into this and they were also upset. I am more upset than the Standing Committee. There has been some delay due to non-response to the tenders, etc. Work has started in Rajasthan; work would be starting in one or two months in Bhubaneswar, in Bhopal, in Patna, etc. This is how the work is distributed and it would take little more time for me.
Even though the previous Government had plans of building six new and improving seven institutions, we are now improving nearly 13 institutions – from seven we are improving 13. Kerala was not in the list of the last Government. After the UPA Government came into power, we included Trivandrum Medical College in Kerala for upgradation and allocated Rs.120 crore. Throughout the country, we have just one slab of Rs.120 crore – Rs.20 crore from the local Government and Rs.100 crore from the Central Government. We try to do that. I would like to assure my friend, Shri Tripathy and others that definitely we are trying to work out the modalities. We had split between the housing and the hospital, etc. to quicken the pace of work. Again, I would like to categorically assure the House that we are definitely into the projects, Sir.
SHRI KHARABELA SWAIN (BALASORE): Mr. Minister, are we to take it as an assurance from you that in Bhubaneswar, the work will start within the next two months?
DR. ANBUMANI RAMADOSS: Within the next three months, it will start in Bhubaneswar.

DR. ANBUMANI RAMADOSS: In three months’ time, the work will start in Bhubaneshwar.

Viswas and others: Please note that besides the six new AIIMS-like institutes the government is taking up the upgradation of 13 exisiting medical colleges to AIIMS standard. Note that some states (UP, J & K and Andhra Pradesh) will have two of their institutes upgraded. Orissa should try to have one of its other medical colleges outside the Bhubaneswar area to be upgraded to that level. The medical colleges in Berhampur and Burla could be possible candidates.
Update: Some more ones are also in the planning stage.

The proposal for setting up of AIIMS-like institutions was approved by CCEA on 16.3.2006. The initial bid process for selection of Project Consultant proved to be unresponsive. The offer of short listed Architectural Design Consultant for preparing enlarged concept design was rejected due to the exorbitantly high price quoted by the selected designer. Therefore, the whole process had to be initiated de novo in December 2006. Several methodological changes were made to ensure progress keeping in mind the complexity of building 6 such institutions simultaneously and lack of capacity for taking the whole project as one. Therefore, it was decided that each AIIMS site would be taken as a separate and independent project instead of clubbing all six together. Project Management Consultant for each site is also being selected for expeditious execution of the work. Ministry has initiated steps to set up dedicated project cell at each site comprising medical expert, engineers and administrative support staff. Each of the projects is being closely monitored by the Ministry to ensure that there is no slippage or delay at any of the sites.

Following is an excerpt from a news report in New Indian Express.
The Government is required to acquire 1.42 acres of private land for construction of the proposed 200-foot wide road that will connect the AIIMS site to the National Highway No. 5.
… Another 2.25 acres of land of the State Police Academy in Patrapada village will be acquired for the project.
The Chief Secretary directed the Home Department to take necessary action and report compliance.
He also directed the General Administration Department to identify the private land falling under the site earmarked for AIIMS under emergency clause.
The meeting was informed that the Orissa State Cashew Development Corporation had already handed over the required land, coming under its area and construction work of a 40-foot wide alternative road along the AIIMS boundary wall to Patrapada village, to Roads and Buildings Division of the Works Department.
The Khurda Collector further reported that the process had been initiated to dereserve 0.803 acre of gochar land coming under the project site.
The Public Health Department in coordination with the Bhubaneswar Municipal Corporation will supply water for the construction works.
A 132-KV double circuit tower existing within the site will be shifted for which the Energy Department will deposit Rs 1.34 crore to the Orissa Power Transmission corporation Limited.
The meeting also decided to take the public-private partnership route for construction of a bus stand, market complex, yatri nivas and other facilities for public convenience.

AIIMS like institutions are to be set up at six places in different parts of the country. For the construction of medical college and hospital complex, the Design Detailed Project Report (DPR) consultants have been selected for all the six sites. DPR would be available by May-June, 2008.
In so far as construction of housing complex is concerned, the bid process for selecting the turnkey developer was carried out for all sites. However, only 2 bids were received, of which one was found responsive. The work has been awarded to developer (M/s. Rajasthan State Road Development Corporation) for Jodhpur. The work relating to construction of housing complex at Rishikesh and Patna has been entrusted to HLL and for Bhubaneswar and Raipur to HSCC, on nomination basis. For Bhopal site, housing complex work would be awarded alongside with the hospital-medical college work.
The state-wise progress regarding construction of AIIMS like institutions can be seen at Annexure.

Following is the press release.
Chief Minister Naveen Patnaik met the Union Health Minister Shri A. Ramdoss today and impressed upon him to start work on the AIIMS Bhubaneswar campus without further delay. The Government of Orissa has already made available land to Government of India for this purpose. The Union Health Minister assured the CM that work would begin latest by January 2008.
Discussion was also held on wide ranging issues in the Health Sector. The Chief Minister requested the Union Health Minister for opening of a Paediatric Centre at Berhampur for treatment of children affected by HIV/ AIDS. He also pursued the request made by the State Government for installation of CD-4 machines in the 3 Medical Colleges at Cuttack, Berhampur and Sambalpur for detection of HIV/ AIDS. The Chief Minister, Orissa requested the Union Health Minister to sanction state-of-the art machines such as Linear Accelerator for the Cancer Hospital at Cuttack. The Union Health Minister assured the CM that all support will be extended for this purpose.
The CM, Orissa pleaded for increase of intake capacity in Dental Surgery (BDS) from 20 to 50. He further suggested opening of MDS course for which necessary posts of doctors have been sanctioned by Government of Orissa. The Union Health Minister promised to consider the proposal favourably.
Discussions were also held for opening new Nursing Colleges in different parts of Orissa in PPP mode. Government of Orissa will take steps to open such colleges shortly.
In related news a Times of India report outlines the progress with respect to the AIIMS clone in Patna. The time line described in that report will most likely also hold for the one in Bhubaneswar.
The bidding for selection of consultant would be opened on October 3, 2007. "The project management committee had approved the expression of interest (EOI) for selection of project management consultants for construction of six AIIMS-like medical college and hospitals in the country," the affidavit stated.
The project management consultants would be selected by December, 2007, and detailed project report would be submitted by April-May, 2008. Civil construction of the hospital and medical college building may be started by early 2008 and completed by 2010. Simultaneously, the manpower requirement for the AIIMS-like institution is being assessed by a separate committee, the affidavit added.

An ambitious Rs 10,000 crore project, led by the Union ministry of health and family welfare, to set up over a dozen multi-speciality hospitals and medical education institutions across the country, under the so-called National Rural Health Mission, will kick off in the first week of October with the opening of architecture and design tenders for six hospital projects.
The six medical institutions —being modelled on New Delhi-based All India Institute of Medical Sciences—at Bhopal, Bhubaneswar, Jodhpur, Patna, Raipur and Rishikesh costing Rs1,500 crore are expected to be completed in 2009. Other phases of the National Rural Health Mission, including super-speciality hospitals and medical institutions in 11 states, will be finalized soon.
This is the first major government investment for health care infrastructure in the country, even as private players such as Fortis Healthcare Ltd, Max Helathcare Ltd, Hinduja Group, Apollo Hospitals Ltd, and Wockhardt Hospitals Ltd, among others, have earmarked between Rs10,000 crore and Rs15,000 crore for new hospitals and expansion of existing ones.
According to an official at the state-owned Hindustan Latex Ltd’s procurement and consultancy division, which has been appointed a consultant to the first phase of the project, the government has received 14 “expressions of interest for the design and architecture of the six AIIMS-like medical institutions proposed in the project.”
IVRCL Infrastructure & Projects Ltd, RITES Ltd, HSCC India Ltd, Hospitech Management Consultants Pvt. Ltd, Technicalia Consultants Ltd, Kotari Associates Ltd, and Borg Lor Associates Ltd are among those that have bid for architecture contracts.
The proposed institutes will be equipped to handle multiple diseases and disorders with 800 hospital beds along with intensive care units and trauma care centres. They will have medical colleges with annual intake of 100 students and will also offer doctoral courses.
The following phases of the National Rural Health Mission, which include upgradation of seven medical institutions in six other states, and setting up of super-speciality medical institutions and research centres in another five states through the public-private partnership model, are also being finalised. The government is also planning upgradation of about 700 hospitals in rural areas especially in underserved states.
The emphasis on health care infrastructure under the five-year, Rs40,000 crore National Rural Health Mission of the government comes in the wake of strong criticism from non-governmental organizations and national healthcare action groups.
