Cabinet approves a much larger estimate for the AIIMS-like institutes and upgradations

2010 Odisha budget has 32.5 crores for adding a medical college to Capital Hospital in Bhubaneswar

The later shows that the 2010 budget has 32.5 crores for making the building and the foundation for the proposed medical college associated with the Capital Hospital. This was proposed and announced several years back. Good to see this in the budget. This is a good and inexpensive way to make a medical college as the hospital, beds, doctors and patients are already there.  Adding a few more as required by MCI would lead to a medical college and at least 100 more students per year would then be able to pursue medicine in a government facility at an affordabale cost. These students and the future PG students would be beneficial to the patients. This is a win-win situation at comparatively small additional cost. (The total budget for this was earlier mentioned as 100 crores.)

This approach should be immediately replicated in all the other larger size hospitals in the state; if necessary in a PPP mode, as is being done in Karnataka. (There was a recent news that a team from Odisha was planing to visit Karnataka to find more about how it is being done there.)

In addition as mentioned before SAIL should do the same with respect to IGH in Rourkela.

Progress report and current thinking on the proposed AIIMS-like institutes

LOK SABHA

The Government has approved setting up of six AIIMS-like institutions in the States of Bihar (Patna), Chhattisgarh (Raipur), Madhya Pradesh (Bhopal), Orissa (Bhubaneswar), Rajasthan (Jodhpur) and Uttarakhand (Rishikesh) under the first phase of Pradhan Mantri Swasthya Suraksha Yojana (PMSSY). It is also proposed to set up two more institutions, one each in the State of Uttar Pradesh and West Bengal in the second phase of PMSSY.

Each institution will have a 960 bedded hospital (500 beds for the medial college hospital; 300 beds for Specialty/Super Specialty; 100 beds for ICU/Accident trauma; 30 beds for Physical Medicine & Rehabilitation and 30 beds for Ayush ) intended to provide healthcare facilities in 42 specialty/super-specialty disciplines.

Bids for construction of hospital and medical college complex for the six AIIMS-like institutions in the first phase are under process. The timeline for completion of these works is the second half of 2012.

This information was given by Minister for Health & Family Welfare, Shri Ghulam Nabi Azad in written reply to a question raised in Lok Sabha today.

Following are excerpts from a report in Indian Express.

… The AIIMS at Bhopal is likely to cost Rs 849.46 crore, Bhubaneswar Rs 820.49 crore, Jodhpur Rs 777.64 crore, Patna Rs 858.43 crore, Raipur Rs 803.84 crore and Rishikesh Rs 820.15 crore.

… the officials suggested that certain operations of these new institutes should be outsourced through Public Private Partnership (PPP). Such operations include outsourcing routine/housekeeping services, hospital information system to a professionally managed agency and outsourcing of nursing orderlies and ward boys and other staff. “This strategy the health ministry officials argued would ensure minimal Group C & D employees,” a Plan panel source said. Nearly 95 per cent of the construction work for residential complexes for Jodhpur and Raipur Institutes has been completed, 45 per cent work has been completed for Patna and Rishikesh, while the construction for Bhopal and Bhubaneswar is gradually picking up,” he said. So far about 152 bids have been received including 57 for hospitals and 95 for medical colleges and hostels. “The technical and financial evaluation would be completed by March 30 and work is likely to commence by June 10,” the source said, quoting health ministry officials.

Sahyog Foundation plans a medicity (with a medical college) in Jagatpur and a medical college in Keonjhar

Following is an excerpt from a report in Business Standard.

Sahyog Foundation, a city-based non-profit organization, would invest Rs 1100 crore in setting up a medicity spread over 300 acres at Jagatpur near Cuttack as well as a medical college and hospital at Keonjhar.

The amount will be raised by the foundation through a mix of debt and contributions from the promoters and Non-Resident Indians (NRIs).

The Keonjhar project which is being taken up on the public private partnership (PPP) mode at an investment of Rs 200 crore will have a medical college with an intake of 50 seats to begin with and a 300-bed hospital.

Debasis Sahoo, director, Sahyog Foundation said, “We will sign the MoU … with the Orissa government for setting up the medical college and hospital at Keonjhar by the end of this month. Apart from alloting 20 acres of land free of cost, the state government has also given us an incentive grant of Rs 10 crore for the project.”

Construction work on the proposed medical college and hospital at Keonjhar is set to take off in April this year and the first batch of students for the MBBS course will be admitted in 2011.

“While the hospital at Keonjhar will offer treatment in various super specialties, the focus will be on malaria ad tuberculosis. Seventy per cent of the beds in this hospital will be reserved for the BPL (Below Poverty Line) families.”

… The medical college and hospital at Keonjhar will offer direct employment opportunities to 3000 people.

For its Rs 900-crore medicity project at Jagatpur, Sahyog Foundation has started the process of land acquisition. The medicity project is expected to be operational by 2014.

Besides a 150-seater medical college and a 700-bed hospital, the medicity will have a homeopathic college, an Ayurvedic college and other public amenities like a shopping mall, gymnasium, swimming pool and food court.

…  Apart from generating around 5000 direct jobs, the medicity will also create indirect employment for 150000 people.

Similar news was reported earlier (for example here) but the numbers in those early reports did not make sense.

Similarly there have been news reports (see here and here) about a US based group called Euthenic Group consortium that wants to set up a 1000 bed super speciality hospital in Orissa. However, after seeing the webpage of Euthenic Group (see http://euthenicgroup.com/) I don’t have a good feeling on this. Also I don’t understand what is meant when expressbuzz says the following:

After completion of the project, it will be handed over to the State Government. The hospital will be the property of the State Government. 

It does not quite make financial sense that this group will make an hospital and then hand it over to the state government at the end of the completion of project. May be by  "at the completion of project"  they mean after a long long time; say 30 years.

160 construction companies bid for the AIIMS-type institutes

Following is an excerpt from a report in expressbuzz.com.

There is an unprecedented response for tenders for the construction of new All India Institute of Medical Sciences (AIIMS)-type medical institutions in Patna, Raipur, Bhopal, Bhubaneshwar, Jodhpur and Rishikesh.

Sources in the Ministry of Health and Family Welfare sais around 160 construction companies joined the bidding making it more competitive.

Thanks to the incentive package, if the construction of the institute is completed within the stipulated time or little in advance of the scheduled time, the company would get benefit of Rs 10 crore at one stroke. As per the detailed project report, construction cost of each institution, which comprises the medical colleges and the hospital, is around Rs 300 crore and for the all the six institutes, the cost would be around Rs 1800 crore.

Gopabandhu Institute of Medical Science & Research, Near Athagarh, Cuttack

I came across this ad in Samaja today.

Looking for more information on this institute I came across the following in the page http://www.thesamaja.com/content.php?key=4.

About 80% of the net profit of The Samaja is spent for the welfare activities of the people of Orissa by way of extending stipend to needy students, by helping the patients and victims of natural calamities and through miscellaneous charity and donations. A large amount goes to the Gopabandhu Institute of Medical Science and Research which is in the verge of completion at Athgarh, Orissa.

 Looking in Orissa links, I found the following in https://www.orissalinks.com/archives/245.

“The Samaja Group (Lok Sevak Mandal)” has shown interest to establish a medical college cum hospital in Cuttack district.

So perhaps Gopabandhu Institute of Medical Science & Research may some day become a medical college.

Just came across the following ad in nijuktikhabar.net. So it seems this institute has or about to start a nursing college.

Union Health Minister has incorporated a strong monetary incentive for fast completion of the new AIIMS-like institutions

Following is an excerpt from a PIB release.

… the Union Health Minister Shri Ghulam Nabi Azad expressed concern over dilution of original role of AIIMS due to heavy patient load. He said for bringing the research and innovation to the fore it is important to decongest the institute. Outlining his strategy for this Shri Azad said that the establishment of 8 AIIMS like institution and upgradation of 19 government medical colleges along with strengthening of health infrastructure at rural, district and state level will reduce the pressure on AIIMS. Speaking with the Journalists after the function the minister said that most of the clearances for the AIIMS like institutions have been procured and civil tenders are being issued. He informed that taking cue from his experience in Jammu and Kashmir where he was the Chief Minister he has incorporated a strong monetary incentive for fast completion of these projects. The Hospital – OPD complex is to be built in 24 months. The Minister informed that incentive upto Rs 10 crore will be given for the early completion of work.

The above tender is about the hospital and medical college. Earlier in May 2008, tenders were floated for the housing complex. In June 2009 it was reported that:

• Out of the 14 blocks of the AIIMS, the roof works of eight blocks have already been completed.
• An alternative road has already been constructed for the Sijua village and 90 per cent work has already been completed.
• Steps have been taken to remove the high-voltage transmission line.

I hope additional progress has been made with respect to the housing construction.

Latest status of the AIIMS-like institutions

Government of India has launched the Pradhan Mantri Swasthya Suraksha Yojana (PMSSY) with the objective of correcting regional imbalances in the availability of affordable/reliable tertiary healthcare services and also to augment facilities for quality medical education and research in the country. Under the Ist phase of this scheme, six new AIIMS-like institutions are being set up, one each in the States of Bihar (Patna), Chattisgarh (Raipur), Madhya Pradesh (Bhopal), Orissa (Bhubaneshwar), Rajasthan (Jodhpur) and Uttarakhand (Rishikesh) at an estimated cost of approximately Rs 820 Crores per institution. Each of these institution will have a 960 bedded hospital (500 beds for the medical college hospital; 300 beds for Specialty/Super Speciality departments; 100 beds for ICU/Accident and trauma. In addition 30 beds for Physical Medicine & Rehabilitation (PMR) and 30 beds for Ayush have been kept. This would be full fledged multi-disiciplinary healthcare institution offering facilities in 42 speciality/super-speciality disciplines. Medical College will have an annual intake of 100 UG intake besides imparting PG/doctoral courses in various disciplines. Nursing College will also have 100 UG intake and 25 PG intake annually.

The respective State Governments have provided 100 acre land free of cost. The Ministry of Health and Family Welfare, Govt. of India engaged eminent architectural firms to prepare the designs and drawings. The architectural/DPR Consultants were selected on global competitive basis. The Project Consultants have also been selected for each site on the basis of open competitive bids. Project Cell at the sites comprising engineering, finance and administrative set up has been constituted and posts are being filled up on deputation basis from other Central Govt./State Govt. organizations.

The construction work has been split into 7 different packages, depending on the functional /sequential requirement and core specialization needs. Tenders/NITs for the main civil packages viz. (i) Civil works for Medical College, Nursing College, AYUSH and Hostels, (Patna, Raipur, Bhopal); and (ii) Civil works for Hospitals – OPD Complex including all internal services and specialized works are being invited. The total cost of these two packages is between Rs.250 – 300 Crore for each site, totaling Rs. 1908 Cr. for all six sites.

The salient features of the tenders /NITs issued on 2nd Nov. are as follows:-

(i) All detailed engineering drawings have been prepared.

(ii) Environmental clearance has been obtained for 5 out of 6 sites and is being obtained for the remaining site (Bhopal).

(iii) Medical College Complex will be built in 15 months time.

(iv) The Hospital – OPD complex is to be built in 24 months.

(iv) Apart from ensuring timely payment, there are very strong incentive clause for early completion with a bonus of 1% of the tendered value per month computed on per day basis, subject to maximum of 5%.

(v) Like-wise there is a strong disincentive clause/penalty if the project is delayed.

Pre-bid Conference is scheduled for 16th November, 2009. Last date of receipt of bids would be 3 Dec 2009
